Appointments are available on weekdays, with additional late afternoon appointments on select Tuesdays, and a weekend clinic on Saturday 9/27. Call or message us to secure your child’s slot.
Please note that we are awaiting state guidance on how to deliver COVID vaccines to children this respiratory season. Updates will be shared here as soon as we know more. Thank you for your patience.

Don’t forget to send your child’s school nurse updated copies of the following:
- Allergy/Asthma Action Plans
- Medication administration forms 💊
While we strive to deliver completed forms to you as quickly as possible, please keep in mind that we are working through many of these requests per day - please give us as much advance notice as possible!
The back-to-school countdown is ON! ⏱️ Most of our patients will begin their academic year the next two weeks. Parents, follow along with us to make sure you are prepared for the first day of school!
(1) Are your child’s immunizations up-to-date? Check state and school requirements and get a copy of your child’s vaccine record to the school nurse.
Puddle Dock patients can log into their patient portal at puddle.pcc.com/portal and download a signed immunization history.

Our newest face at the front desk: Catherine! ✨ She has spent the past 8.5 years working in pediatric primary care administration, where she’s enjoyed building relationships with children and their families and helping create a positive experience for every patient who walks through the door. Queen of all things on the admin side of Puddle Dock, she takes pride in helping our office run smoothly so patients receive the best possible care.
FUN FACT: Catherine just achieved her goal of hiking all 48 of NH’s 4000’ mountains! ⛰️
